Step 2: Office Visit to:

-Complete package/ completar paquete
-Paperwork Drop Off: / Llevar documentos:

➤Birth Certificate / acta de Nacimiento 

➤Immunization records / cartilla de vacunas

➤Parents ID / Identificacion de padre(s)

➤3 Bills as proof of address dated within the last 2 months / 3 facturas como prueba de direccion con fecha de los ultimos 2 meses
